The provider had systems in place to assess people’s needs, however they were not robust and did not ensure people were involved. People’s communication needs were outlined in their care plans.

Some care plans and some risk assessments were not fully reflective of people’s needs. For example, 1 person’s care plans and risk assessments lacked information regarding diabetes management and another person’s lacked information regarding their risk of choking. This meant staff did not have enough information to support the person safely. The provider took immediate action, and we saw no evidence of harm to people.

Staff told us that they did not always feel that information was shared between shifts which could put people at risk. One staff member told us, “Handovers are not always completed, and information is not always shared with us.”
