Well-led
Well-led – this means we looked for evidence that leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant the management and leadership was inconsistent. Leaders and the culture they created did not always support the delivery of high-quality, person-centred care. The service was in breach of the legal regulation relating to governance.

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities. The service had a clear vision and strategy staff understood and worked towards. We observed staff were respectful, caring and passionate about providing good quality care to people living at Rosebank Care Home. One staff member said, “our aim and goal is to treat everyone as an individual and treat everyone at the end of their life how they would like to be treated.” Staff told us they felt listened to and empowered to work with the managers to run the service. One staff member told us, “There is always a go to person. They are open and transparent, once a week [Operations manager], she is here and has an open door. [Registered manager] has an open-door policy.”

Governance, management and sustainability
The service did not have clear systems of accountability and good governance. The provider carried out regular audits of the service and had weekly clinical meetings to discuss actions and trends, however their audits failed to identify shortfalls in relation to restricting peoples’ access to their bedrooms. This meant the governance systems did not ensure they were working in line with the mental capacity act.
The provider’s policy required staff supervisions to be carried out a minimum of 4 times per year and for staff to remain up to date with mandatory training. The registered manager failed to ensure staff supervisions and mandatory training were up to date. This meant people were being supported by staff who did not always have sufficient training and support. The registered manager told us the shortfall in supervision oversight was due to senior staff sickness absence. To mitigate this the registered manager told us she had since implemented a new supervision form to enable her to do more frequent supervisions and was training more senior staff, including the newly appointed head of care, to carry out supervisions.
At the time of the assessment, call bell audits were not, and had not been taking place, because the technology used by the service did not facilitate analysis of call bell response times. However, the provider had ordered a new call bell system and created a call bell audit template which would be operational by late January 2025.
The provider took proactive action towards sustainability. At the time of the assessment the service was finalising plans to implement plant-based cleaning products designed for care homes. This was due to be rolled out in 2025.
Where required, the provider submitted notifications to the CQC and made appropriate referrals to relevant stakeholders.
