Well-led
The trust had recently made changes in leadership at the service. Appropriate processes were in place to assist staff and leaders track and monitor audits. However, appropriate processes were not in place to ensure all patient physical health care needs were communicated effectively to the physical health care team resulting in patient needs not always being met. We found managers and leaders had failed to address areas of improvement identified at the assessment completed in September 2024.
