At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

Score: 3

The provider made sure people’s care was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. Staff met with people, ideally before their care package began, to discuss their care needs and how they would like to be supported. This information was then captured in a comprehensive care and support plan which clearly informed staff what support was required. This information was regularly reviewed and updated at predetermined intervals and in-between as necessary. Care workers who knew people well were able to feed into this process, so the smaller details were able to be captured about people’s preferences and how they liked to be supported.

People and their relatives felt informed in their care assessment and the care planning process. Comments we received included, “Mum came out of hospital on the [date], [Staff member] came on [date] to have a discussion with mum,” “The initial assessment, they came and talked about mum. Then they came back when it went to seven days,” and “We had a lady here only a couple of weeks ago, to see if everything was alright.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. Care records were clear about whether people had the capacity to consent to their care and if they didn’t, who consented on their behalf. Staff were aware of the importance of adhering to the Mental Capacity Act 2005 and ensuring they had people’s consent prior to supporting them.

People and their relatives confirmed staff did not provide support without their consent. They told us, “[The care workers] talk to her and explain what they are doing at that time,” and “It’s all done in a consensual way. They are very friendly with mum, they explain what they are going to do. There is a countdown before she stands up.” A care worker said, “I communicate with [the person receiving care] to let them know what I want to do and once they are in agreement I do it.” Another care worker told us when supporting people, “I ask them. They can say no and I have to respect their decisions.”
