Well-led
We found there was an open and honest culture where staff felt they were freely able to speak up. Staff felt supported by managers in their career progression and managers ensured they kept up to date with best practice and guidelines. There were numerous examples of learning from the previous inspection.
Although the service now held regular, formal governance meetings that were documented and completed an audit schedule. There was a lack of oversight and documented follow up of risks and actions from governance meetings. Therefore, the service was in breach for Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 Regulation 17 (2)(a). This states services must “monitor progress against plans to improve the quality and safety of services, and take appropriate action without delay where progress is not achieved as expected”.
There was a lack of documentation and follow up of actions, which meant expectations were not readily available, benchmarked or easily monitored. Therefore, the service was in breach for Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 Regulation 17 (2)(a).
