Effective
We found this practice was not providing effective care in accordance with the relevant regulations.
During our assessment of this key question, we found concerns related to
the completeness of patient care records and
the practice having an ineffective system to keep dental professionals up-to-date with current evidence-based practice.
These concerns resulted in breaches of Regulations 12 (Safe care and treatment) and 17 (Good governance) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014.

Assessing needs

Delivering evidence-based care and treatment
The practice had some systems to keep dental professionals up-to-date with current evidence-based practice.
Improvements were needed to clinician’s understanding of the guidance on periodontal disease and the management of periodontal patients, guidelines for antibiotic prescribing and the concept of Gillick competency.
We looked at the dental care records for 6 service users and found that not all records were present. Missing details included an updated medical history, intraoral and soft tissue checks, extraoral examination, Basic Periodontal Examination, risk assessment, diagnosis/prognosis, and X-ray justification, grading and reporting.
The practice did not have effective systems for the safe prescribing of medicines. There was no log in place for antimicrobial prescribing and where prescribing was not in line with the current guidance, a rationale for prescribing outside of the guidance was not recorded. Antimicrobial prescribing audits were not carried out.
Staff worked together and with other health and social care professionals to deliver effective care and treatment.
The practice had systems for referring patients with suspected cancer under the national Faster Diagnosis Standard.
