Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question good. At this assessment the rating has remained good.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. The registered manager had a process in place to review accident and incidents, these were responded to appropriately and lessons were learnt to drive improvements in the service. Staff told us they knew how to report concerns and they were involved in lessons learnt. Comments from staff included, “We would have meetings to discuss lessons learned” and “We have regular meetings to discuss any issues or concerns.”

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Assessments of people’s needs gave good direction to staff, this ensured people were transitioned safely into the service. Staff told us they would engage with people and their families and contact the relevant professionals to support them if people’s needs had changed.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff received safe guarding training and were clear about their responsibilities in responding to and reporting any safeguarding concerns. The provider shared concerns quickly and appropriately, analysis and trends were completed with action plans and lessons learnt. People told us the service was safe, comments from people included, “Definitely I feel safe with them [Carers]”, and “Yes, it is very safe, I can say that every one of them [Staff] are brilliant.”

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Risk assessments were person centred and regularly reviewed to ensure they contained accurate up to date information. Staff told us they had time to read people’s risk assessments and would always involve people in planning their care. People told us their care plans and risk assessments were regularly reviewed and met their needs.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Comments from staff included, “We have environment risk assessments and if I was going to the property for the first time I would always read them”, “Equipment is always serviced, stickers are placed on them and if we had any concerns we would contact the office.” We received positive comments from people about their care, which included, “I have been having their care {A&B Healthcare] for a number of years and all the staff are brilliant” and “Yes, it is very safe, nice, lovely carers, excellent support from them.”

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. Staff told us there had been improvements in regard to staffing and there was enough staff to care for people safely. Comments from people included, “Staff are very good, they all have a positive attitude and are never late”, and “Staff always arrive on time and their timings are tailored to my condition, they [Staff] are all good and have never been late.” Staff were recruited safely.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. Staff understood how to protect people from infection by wearing the correct protective equipment. People told us staff wash their hands and wear gloves. One person said, “Overall, I am very please with them [Staff], they wash their hands and wear gloves.”

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, and preferences. People were supported by staff who followed systems and processes to prescribe, administer, record and store medicines safely. Staff involved people in planning, including when changes happened. Audits were completed by the provider and staff were up to date with medication training and had their competencies assessed. People were happy with the support they received with their medicines, one person told us, “My carer spotted a problem with my medication and contacted the chemist and got it all sorted out for me.”
