Effective
We assessed and inspected against one quality statements; assessing needs. During our assessment of this key question, we found staff involved people in decisions about their care and treatment and provided them advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. People with long-term conditions received effective care and treatment in line with national guidelines.
Our review of clinical searches of patient records showed that some areas of care was not delivered in line with best practice guidance. However during the on site inspection this had been rectified and new safeguards put in place so care and treatment plans would be managed effectively moving forward.
The practice monitored uptake of childhood vaccinations and cervical screening, we identified that the practice was in line with local and national averages. We also found that Do Not Attempt Cardio-Pulmonary Resuscitation (DNACPR) and Recommended Summary Plan for Emergency Care and Treatment (ReSPECT) records had been completed in line with national guidance and the Mental Capacity Act.

Assessing needs
The feedback we received from patients was limited regarding assessment of needs. However, the information we did receive was positive regarding patient’s needs being met. Results from the National GP Patient survey published in July 2024 showed 94% of respondents stated that during their last GP appointment they had confidence and trust in the healthcare professional they saw or spoke to.
The practice coded patients to identify additional support within the care records system, such as the requirement for longer appointments or for a translator to be present. Staff checked people’s health, care, and wellbeing needs during health reviews.
Clinical staff used templates when conducting care reviews to support the review of people’s wider health and wellbeing. The provider had effective systems to identify people with previously undiagnosed conditions. Staff could refer people with social needs, such as those experiencing social isolation or housing difficulties, to a social prescriber.
A remote review of the patient record system showed that patients received appropriate long-term condition reviews. Patients with long-term conditions were offered an effective annual review to check their health and medicines needs were being met. For patients with the most complex needs, the GPs worked with other health and care professionals such as Pharmacists and to deliver a coordinated package of care. Registers were kept of patients with different health requirements. The practice identified patients with caring responsibilities and had signposting and policies in place to support their needs.
People were invited for appropriate health checks. At the time of the inspection the practice has achieved 98% uptake for cervical screening target. The practice had worked closely as a team with Saturday Nurse appointments available to help increase uptake.
