Responsive
We inspected 2 quality statements for this key question. We found staff had been working incredibly hard to help improve the outcomes for children. We found that significant improvements had been made as detailed in this report.

Person-centred Care
During our last inspection we found not all children had a Comprehensive Health Assessment Tool (CHAT) completed and information from these and from community services had not always been used to inform care. In addition, children had not been involved in the development of their care plans to ensure their views and wishes were reflected.
During this inspection we found all children had a timely CHAT assessment completed. Systems have been improved to ensure more effective monitoring of this to ensure they were timely to help identify children’s needs. Children were seen a minimum of 3 times per day by a clinician and after incidents to ensure any immediate concerns and needs were addressed.
We found each child had a named nurse to oversee and coordinate their care. Named nurses were responsible for the development of any required care plans for the child and help create a stronger rapport promoting better understanding of the child’s needs and wishes. However, further development was needed to ensure the child’s voice was reflected in their care plans. Nurses had access to additional information about the child called ‘My Story’ to help ensure they had as much information about the child to underpin their relationship with them.

Equity in access
During our previous inspection we found that children did not have access to immunisation and vaccinations and there was a lack of monitoring for children with a long term condition.
During this inspection, we found improvements had been made. Systems were in place to ensure children with long term conditions and those needing additional monitoring were cared for in line with national guidance thus, helping to monitor their conditions. Children were now able to access appropriate immunisations and vaccinations minimising the risk of communicable diseases.
