Well-led
We looked for evidence that service leadership, management and governance assured high-quality, person-centered care; supported learning and innovation; and promoted an open, fair culture. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same. Freedom to speak up
Staff told us that they understood how to raise concerns and provide feedback within the service. They also told us they understood what whistleblowing was. Leaders in the service told us that they tried to encourage an open culture, where staff would feel confident to raise concerns and that they would listen and act where appropriate on feedback from staff. Staff described examples where they had raised concerns, such as GP workload and the practice had responded by amending the total number of appointments per session for GPs. Staff told us they felt supported by line managers and other leaders within the service. They also told us they would feel comfortable raising concerns and ideas with managers at work and that if concerns were raised the practice would address them. Staff described opportunities to provide feedback in meetings and appraisals and felt they could approach managers at any time.
The service fostered a positive culture where people felt they could speak up and their voice would be heard. The practice had a whistleblowing policy in place and there were clear processes for staff to speak up and which provided staff with a contact outside the practice to whom they could raise concerns if they did not feel comfortable raising concerns internally.

Governance, management and sustainability
Staff described a clear understanding of their roles and responsibilities and that of others. They were encouraged to speak up and where needed, discussed concerns openly and were involved where possible in finding solutions. Staff told us there was a friendly and supportive environment to work in and they felt supported by the whole team. Staff also told us that managers and GPs were visible and approachable.
There were systems of audit and appraisal to support performance and development. The provider had established governance processes that were appropriate for their service. Staff could access all required policies and procedures The practice had set out clear responsibilities, roles and systems of accountability to support good governance. They acted upon information about risk, performance and outcomes, and shared this securely with others when appropriate. They used this information to manage and deliver good quality, sustainable care, treatment and support.
