Well-led
We did not inspect the well-led key question in full as part of this focused inspection. We looked at 1 quality statement for this key question and reviewed the breaches from the last inspection. We found managers had addressed concerns with the out of hours provider and staff now utilised the contracted provider for out of hours prescribing.

Governance, management and sustainability
At our last inspection we found the provider did not utilise the contracted out of hours service (a service in place for healthcare staff to seek advice or treatment for patients overnight) based on previous challenges with the provision and was yet to gather evidence to escalate this. During this inspection, we found that managers had addressed concerns with the out of hours provider through regular communication, and staff now utilised the contracted provider. We were told at this inspection there were no outstanding issues with the out of hours provider and the process was followed by staff and working well.
During our last inspection we found there was no monitoring of night shifts to ensure risks were identified and managed appropriately, and managers had not identified the concerns raised regarding first night risks to patients. At this inspection, we were informed that performance issues regarding night shifts had been addressed and regular agency staff now covered night shifts. A clinic coordinator now monitored night staff performance and activity with regular audits to ensure patients received appropriate care.
