Governance, management and sustainability

The practice had a governance system that included policies and procedures, which were accessible to staff and were reviewed on a regular basis.

The provider had recently taken over the practice and had implemented a number of changes to support governance in a short period of time. This included signing up with a compliance support provider and engaging a compliance advisor as well as transferring to electronic patient records. At the time of the inspection the practice was in transition between paper based and the compliance portal documentation.

Systems and processes were mostly embedded, and the inspection did not highlight any significant issues or omissions. The provider was very receptive to our feedback and areas requiring improvement were acted on immediately.

Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and records were stored securely and complied with General Data Protection Regulations. The practice had recently introduced the use of an electronic device to enable patients to update personal information and medical history.

There were effective processes for identifying and managing risks, investigating incidents and accidents, and for receiving and acting on safety alerts.

The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance.

Staff had carried out a disability access audit and had formulated an action plan to continually improve access for patients. The practice was wheelchair accessible with step free access to 1 ground floor surgery. They could provide information in large print or alternative formats. The practice had a hearing induction loop to support patients with a hearing impairment and staff had access to language translation services if required.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement. Information about how to complain was made available to patients in a patient folder at reception.

Staff had clear responsibilities, and systems of accountability to support good governance.

Staff feedback was obtained through meetings, surveys, and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate.

Feedback from patients, the public and external partners was collected to which the practice responded.

The practice had taken steps to improve environmental sustainability. For example, by the segregation of waste and the introduction of electronic patient care records.
