Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People had communication plans in place. We saw staff understood how to effectively communicate with each person. Information was accessible to people including easy read documents for one person.

People and relatives were involved in planning and reviewing their care and support. Their views and opinions were respected, listened to and implemented as part of the day-to-day support.

Consideration was given to where and how the reviews were carried out to ensure people were comfortable, relaxed and enabled to make decisions about their care and treatment. For one person the manager set up an informal catch up and chat to put the person at ease and make them feel more relaxed. An annual review we looked at was detailed and reflected the wishes of the individual including things they wanted to achieve in the next 6 months.

Relatives told us communication with them was good. Comments included, “[Staff] keep me up to date on what [family member] is doing and we have 6 monthly review meetings also” and “[Staff] would let me know if [family member] was unwell and tell me what they’re doing. Communication is very good from them. We also have regular meetings, so I can tell them my thoughts”.

An electronic care system had been introduced 6 months previously and care plans and risk assessments were in place. Further information was being added from the paper care records.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

People were able to choose and decide how they spent their days and filled their time. Care records included information about people’s capacity and how either through verbal or non-verbal means they were able to refuse or give consent for specific decisions. Mental capacity assessments and best interest decisions were in place where people lacked capacity for a specific decision.
